Do I need to see my doctor first?

In New Jersey, due to direct access, you do not need a referral from your doctor.  We however will work very closely with your doctor and can obtain a referral if needed.

What should I expect from my FIRST Physical Therapy visit at Align Physical Therapy & Wellness?

Your first visit at our clinic will begin with a 1:1 evaluation with the physical therapist.  The Physical Therapist will perform a complete evaluation including reviewing any relevant past & present medical history, physical examination and assessment, and manual hands-on therapy.  During your visit the therapist will work with you to develop a plan of care to achieve your goals.  You will leave the clinic feeling better!

What should I expect from my Physical Therapy treatment?

After your first/initial visit, you and your therapist will follow the plan of care.  The physical therapist will implement various treatment interventions including specific exercises, manual therapy techniques, functional training, and other pain management techniques.  These interventions will assist with pain relief, improve functional mobility and provide you with specific techniques & education to maintain and achieve your goal.

What should I bring with me?

Before your first visit, please complete the forms posted under "Patient Forms" in the "For Patients" section. 
 
To your first visit please bring:
 
Completed patient intake forms
Referral or Prescription from your Doctor (if applicable)
Photo Identification (Driver's License or State ID)
Insurance Card (if applicable)
 
**You will be able to complete your patient intake form at the clinic IF you are unable to complete it before your first visit**

What should I wear?

Loose-fitting, comfortable clothing is recommended.
 
*NO open-toed shoes, flip-flops, sandals

Do you accept insurance?

Accepted Answer

We accept most major health insurance plans.  We also accept Worker’s Compensation and motor vehicle accident (MVA) claims. 
If we do not take your insurance or if you choose not to use your insurance or if you are uninsured, we offer a competitive cash rate.

Will Physical Therapy be covered under my insurance?

Accepted Answer

Majority of all health insurance companies will cover Physical Therapy services.  We will however verify your insurance to provide you with the details of your coverage which will include the number of allowed visits, co-payment, and the deductible information.
